Enhance your knowledge and skills with accurate, reliable information to confidently engage adults in simple care planning conversations to complete a Healthcare Proxy and Personal Directive (Living Will). We provide proven strategies and multilingual tools to have seamless conversations from simple to managing chronic illness and living well with serious illness.

Webinar includes:

  • HCM Health Care Planning Process: key conversations and accompanying documents;
  • Review of five MA care planning documents, tools and conversation guides;
  • Review the Getting Started Tool Kit;
  • Introduce the Next Steps Tool Kit and guides for chronic and serious illness conversations; and
  • Strategies to help adults subject to Guardianship explore possible alternatives.

Part 2

Simple to Serious Illness Conversations

Care professionals can help adults build on planning conversations to live well with chronic and serious illnesses. Learn how to start simple planning conversation with consumers to make a personal care plan using the HCM Getting Started and Next Steps Tool Kit, and flow into a more complex conversations using Ariadne Labs Serious Illness Conversation Guide.

The training includes:

  • Review of MA Law: Your right to direct your care decisions and make a plan;
  • Brief review of the HCM Getting Started Took Kit;
  • The HCM Next Step Tool Kit;
  • Review of five MA care planning documents and handy discussion guides;
  • Introduce the Ariadne Lab's Serious Illness Conversation Guide’s structured communication approach and discussion guide; and
  • Resources and tools.

Participants receive Honoring Choices MA Health Care Planning Ambassador Certificate, and access to no-cost tools kits, videos, multilingual documents and conversation guides.
